As you requested, I am including a complete list of the files I want (attached to this form) - I remembered to give you a list of "extra" files to add in case there's additional space on the DVD's, and I also remembered to give you a list of files to cut should you run out of room.

My payment is enclosed, and is in the amount shown in the final table, under "Subtotals #1 thru #4". I am sending you either a cheque drawn on a Canadian bank, a money order, a bank draft, or cash (even though you don't recommend sending large quantities of cash through the mail) - no coins. I understand that if I send you a cheque drawn on a non-Canadian bank, my order may be delayed by up to six weeks until the cheque clears.
